Notice of Office Closure
Dear GFP Patients:
It is with a heavy heart that we announce the closure of Generations Family Practice.
Our final day of patient care will be: 12/22/2017.
It has been a great pleasure providing for the health care needs of our community over the last 19 years. This has been an extremely difficult decision to make. Unfortunately, with the current turmoil in Primary Care and healthcare overall, we find ourselves unable to continue operations.
Your medical records are confidential, and a copy can only be released with written permission. However, as a part of the Privia Medical Group, your current release grants Privia group providers access to historic records when you initiate care at their offices. While you have many options in Northern Virginia, these excellent Family Physicians are located nearby, are within the Privia network, and are accepting new patients.

If you choose to transfer to a non-Privia practice, after establishing care at a practice, a signed authorization form will be required to transfer your records. Requests made after 12/22/2018 may be sent to manager@generationsfp.com. Please note, there will be a delay on processing requests in mid-late January for approximately 8 weeks as Athena converts our records to a digital file for future access.
